Abstract
Infectious diseases remain a major cause of morbidity and mortality worldwide, with particularly significant challenges arising from emerging infections, tropical diseases, sepsis, antimicrobial resistance, and healthcare-associated infections. This chapter provides a comprehensive overview of infectious diseases, tropical medicine, critical care, sepsis, antimicrobial stewardship, and vaccination, integrating principles of clinical assessment, microbiological diagnosis, evidence-based treatment, prevention, and public health practice. Common bacterial, viral, fungal, and parasitic infections are discussed with emphasis on their epidemiology, transmission, clinical manifestations, diagnostic evaluation, and appropriate antimicrobial therapy. Tropical medicine encompasses important conditions such as malaria, dengue, enteric fever, leptospirosis, rickettsial infections, neglected tropical diseases, and other geographically relevant infections.
Particular attention is given to critically ill patients with severe infections and sepsis. Early recognition of organ dysfunction, hemodynamic assessment, appropriate microbiological sampling, timely antimicrobial therapy, fluid and vasopressor management, respiratory support, source control, and intensive monitoring form essential components of sepsis management. Principles of critical care are integrated with the management of shock, respiratory failure, multiorgan dysfunction, and other life-threatening infectious complications.
Antimicrobial stewardship is emphasized as a fundamental strategy for combating antimicrobial resistance through appropriate antimicrobial selection, dosing, route, duration, de-escalation, and surveillance while minimizing unnecessary exposure. Infection prevention measures, including hand hygiene, isolation precautions, environmental control, and prevention of healthcare-associated infections, are also addressed. Vaccination is discussed as a cornerstone of preventive medicine, incorporating routine immunization, adult and high-risk vaccination, travel-related vaccines, and strategies to improve vaccine coverage. An integrated clinical, microbiological, critical care, and public health approach is essential for reducing infection-related complications, antimicrobial resistance, healthcare burden, and preventable mortality.
