Abstract

Medical mycology is an important branch of clinical microbiology concerned with the detection, isolation, identification, and laboratory diagnosis of fungi responsible for human infections. The increasing incidence of superficial, cutaneous, subcutaneous, systemic, and opportunistic mycoses, particularly among immunocompromised individuals, has emphasized the need for accurate and timely fungal diagnosis. This chapter provides a comprehensive overview of the fundamental laboratory approaches used in the diagnosis of medically important yeasts, molds, and dimorphic fungi. Appropriate collection, transportation, processing, and preservation of clinical specimens are discussed as essential pre-analytical steps for obtaining reliable results. Direct microscopic techniques, including potassium hydroxide (KOH) preparation, Gram staining, lactophenol cotton blue examination, and other specialized fungal staining procedures, are described for demonstrating fungal structures in clinical specimens and cultures. Methods of fungal isolation and cultivation using appropriate media, incubation conditions, and aseptic techniques are presented. Identification of fungal isolates based on colony morphology, pigmentation, microscopic structures, yeast characteristics, germ-tube testing, biochemical reactions, and other conventional methods is discussed systematically. Modern diagnostic approaches, including antigen detection, serological assays, automated identification systems, MALDI-TOF mass spectrometry, and molecular techniques, are also introduced. Particular attention is given to differentiating colonization or contamination from clinically significant fungal infection. Quality assurance, biosafety, antifungal susceptibility testing, contamination prevention, and interpretation of laboratory findings in correlation with clinical information are emphasized. Integration of conventional and advanced diagnostic techniques facilitates rapid and reliable identification of fungal pathogens and contributes to effective clinical management of mycoses.
