Abstract

Molecular genetics and genomic technologies have revolutionized biomedical research and clinical practice by enabling comprehensive analysis of genes, genomes, and their roles in health and disease. Molecular genetics focuses on the structure, function, regulation, and expression of genes at the DNA and RNA levels, providing insights into the molecular mechanisms underlying inherited and acquired disorders. Rapid advances in genomic technologies have facilitated high-throughput analysis of genetic information, allowing clinicians and researchers to identify disease-associated variants, understand complex biological pathways, and develop personalized therapeutic strategies. These innovations have significantly enhanced the diagnosis, prognosis, and management of genetic disorders, cancers, infectious diseases, and multifactorial conditions.

This chapter introduces the fundamental principles of molecular genetics, including DNA replication, transcription, translation, gene regulation, mutation mechanisms, and epigenetic modifications. It also examines the major genomic technologies that have transformed modern medicine, such as polymerase chain reaction (PCR), Sanger sequencing, next-generation sequencing (NGS), whole-genome sequencing (WGS), whole-exome sequencing (WES), transcriptomics, microarray analysis, and CRISPR-Cas genome editing. The integration of bioinformatics, computational genomics, and artificial intelligence in genomic data analysis is also highlighted as an essential component of precision medicine.

In addition, the chapter discusses the clinical applications of molecular and genomic technologies in prenatal diagnosis, newborn screening, oncology, pharmacogenomics, infectious disease surveillance, and rare disease diagnostics. Ethical, legal, and social considerations—including genomic data privacy, informed consent, incidental findings, and equitable access to genomic medicine—are also explored.
