Abstract
Quality management, clinical governance, and continuous healthcare improvement are fundamental to delivering safe, effective, patient-centered, and efficient healthcare services. This chapter examines the principles and practical approaches used by healthcare organizations to establish, monitor, and continuously improve standards of clinical and administrative performance. It explores the major dimensions of healthcare quality, including patient safety, clinical effectiveness, timeliness, efficiency, equity, accessibility, and patient experience. The concept of clinical governance is discussed as an organizational framework that promotes accountability through evidence-based practice, clinical audit, risk management, professional development, ethical practice, and transparent decision-making. Key quality-management approaches such as Total Quality Management (TQM), Continuous Quality Improvement (CQI), Plan–Do–Study–Act (PDSA) cycles, Lean management, Six Sigma, benchmarking, and key performance indicators are examined in the context of hospitals and other healthcare institutions. The chapter also addresses incident reporting, root cause analysis, clinical audits, patient feedback, mortality and morbidity reviews, and corrective and preventive actions as mechanisms for identifying gaps and reducing preventable harm. Particular emphasis is placed on developing a culture of safety, multidisciplinary teamwork, leadership commitment, staff participation, and data-driven decision-making. Digital technologies, electronic health records, healthcare analytics, dashboards, and artificial intelligence are increasingly enabling real-time monitoring of quality indicators and early identification of potential risks. By integrating clinical governance with systematic quality-improvement strategies, healthcare organizations can enhance accountability, optimize processes, reduce errors and unwanted variation, strengthen patient trust, and achieve sustainable improvements in clinical outcomes and organizational performance.
